The Surface Water Application Form is used to request authorization to abstract or use water from rivers, lakes, streams, or other surface water sources. It collects basic applicant information, the intended purpose of water use, details of the water source and location, the volume of water required, and the method of abstraction. The Groundwater Application Form (Form B1), issued under Malawi’s Water Resources Act (Cap. 72:03) and Water Resources Regulations 2018, is used by individuals or organizations to apply for permission from the National Water Resources Authority (NWRA) to drill, construct, or use groundwater sources such as boreholes, wells, or springs. The form collects details about the applicant, land ownership, location, water source, intended use, and technical borehole specifications, and must be submitted with supporting documents like maps, borehole reports, and proof of payment.
The Effluent Discharge Permit Application Form is used to apply for permission to discharge sewage, trade waste, contaminated water, or other effluents into the environment. The form captures details about the applicant, the location of the discharge, the nature and volume of effluent, method of discharge, technical specifications, and the receiving water body or land. Applicants may also be required to provide sampling points, wastewater treatment information, and chemical composition data where applicable. Supporting documents and proof of payment must be attached before submission.
The Borehole Completion Form is used to record all technical and operational details of a newly drilled or developed water point. It includes information on the location and ownership of the borehole, the drilling method and construction materials used, pump installation data, hydrogeological conditions, water quality test results, and yield or pump test performance. This form ensures that the borehole meets required standards and provides an accurate record for monitoring, maintenance, and future water management.
